  • 摘要: 肝移植作为终末期肝病唯一有效的根治手段,历经近六十载发展,已从高风险的探索性手术转变为成熟的常规治疗。本文系统回顾了肝移植手术技术的发展历程,重点介绍了肝切除联合Ⅱ~Ⅲ段部分肝移植的延期全肝切除术和自体肝移植技术革新、微创与机器人技术应用及异种肝移植技术突破等关键领域的创新,剖析这些肝移植技术的具体术式特点、临床应用优缺点及存在的问题。同时,结合中国肝移植特色与贡献,探讨了当前肝移植面临的挑战与未来发展方向。精准化、微创化、边缘供肝高效利用及异种供肝的临床应用将引领肝移植手术进入全新时代。

     

    Abstract: Liver transplantation, as the only effective radical treatment for end-stage liver disease, has evolved from a high-risk exploratory surgery to a mature routine treatment over nearly six decades. This article systematically reviews the development of liver transplantation surgical techniques, focusing on innovations in key areas such as the deferred total hepatectomy combined with partial liver transplantation of segments Ⅱ to Ⅲ, technical advancements in liver autotransplantation, application of minimally invasive and robotic techniques, and breakthroughs in liver xenotransplantation. It analyzes the specific surgical characteristics, clinical application advantages and disadvantages and existing problems of these liver transplantation techniques. Meanwhile, combining the characteristics and contributions of liver transplantation in China, it discusses the current challenges and future development directions of liver transplantation. Precision, minimally invasiveness, efficient utilization of marginal donor livers, and the clinical application of xenogeneic donor livers will lead liver transplantation surgery into a new era.
