  • 摘要: 肝移植是目前治疗终末期肝病的有效手段, 但器官的短缺问题也日益突出。心脏死亡器官捐献(DCD)是现阶段缓解器官短缺的一种重要手段。由于DCD供体发生脑死亡或心脏停搏时出现的一系列病理生理变化, 会导致供体器官功能损害, 因此供体器官功能维护直接关系到捐献成功率和受体的安全。通过选择合适的捐赠者(Maastricht分类Ⅲ型)、高质量的供体和匹配的受体(高风险供体向低风险受体), 使用先进的器官保存技术(如低温机械灌注、常温机械灌注及全身静脉系统氧气灌注)以及药物控制, 可使DCD供体的功能得到维护从而获得良好的移植效果。
